Student-athletes can submit claim documents by mail, fax or email as shown on the PDF ID Card.
The fastest and easiest way is to scan the documents into their computer and use the following secure upload link:

Students need to make sure that the documents are readable and the claimant’s name is clearly evident on documents uploaded.
Otherwise, students can submit as follows:
