This form
is due at the March 30 Senate meeting.
Monthly Organization Report Form
This
form must be
submitted by the last meeting of each month.

Organization Registration Form
At the
beginning of each year, each organization must submit its constitution
to Senate for approval by the Policy Committee, along with a
registration form, which can be downloaded here:

You may either
fill this form out using Excel, then print it out to turn in, or you may
print it out, then fill it out to turn it in. If you fill it out
in Excel, you may e-mail the completed form to
ssenate@gmail.com. To see

Event Evaluation Form
After each
event which received money from Senate, an event evaluation is required.
It is to be turned in to the Treasurer along with copies of receipts
from the event. The Evaluation Form can be downloaded here; please
fill it out and send it to
senate.allocations@gmail.com.

Deadline Extension Form
If you are
unable to get your receipts or event evaluations turned in by the two
week deadline, a Deadline Extension Form must be filled out
electronically and submitted to
senate.allocations@gmail.com.

Prize Signature Form
The newly
adopted procedures for giving away prizes require signatures of those
who receive prizes. For your convenience, Student Senate provides
